益肾化瘀助孕方治疗肾虚血瘀型多囊卵巢综合征的临床研究

Clinical Study on the Effect of Yishen Huayu Zhuyun Formula on Polycystic Ovary Syndrome of Kidney Deficiency and Blood Stasis Type

    目的 研究益肾化瘀助孕方治疗肾虚血瘀型多囊卵巢综合征的临床疗效及对患者血清炎性因子及性激素水平的影响。
    方法 纳入2022年5月至2023年7月在新余市中医院就诊的肾虚血瘀型多囊卵巢综合征患者，随机分为对照组和观察组各78例，对照组采用西医常规治疗，观察组在对照组治疗基础上加服益肾化瘀助孕方，2组均治疗3个月经周期。比较2组治疗前后中医总证候积分、子宫螺旋动脉阻力指数（RI）、排卵日子宫内膜厚度改善情况；比较2组治疗后妊娠情况；检测2组治疗前后血清硫酸脱氢表雄酮（DHEAS）、转化生长因子-β1（TGF-β1）、胰岛素抵抗指数（HOMA-IR）、白介素-6（IL-6）、性激素结合球蛋白（SHBG）及血清促黄体生成素（LH）、促卵泡激素（FSH）、雌激素（E2）水平变化。
    结果 治疗后，2组患者中医总证候积分、RI降低，排卵日子宫内膜厚度升高，观察组优于对照组（P<0.05，P<0.01）；2组患者血清DHEAS、TGF-β1、HOMA-IR、IL-6降低，性激素结合球蛋白（SHBG）升高，观察组优于对照组（P<0.05，P<0.01）；2组血清LH、FSH降低，E2升高，观察组优于对照组（P<0.05，P<0.01）；治疗后，观察组排卵率、妊娠率均高于对照组（P<0.05）。
    结论 益肾化瘀助孕方能显著减轻肾虚血瘀型多囊卵巢综合征患者的炎症状态，调控性激素水平，临床疗效显著。

     

    Abstract:
    OBJECTIVE To study the clinical efficacy of Yishen Huayu Zhuyun Formula on polycystic ovary syndrome of kidney deficiency and blood stasis type and its effect on serum inflammatory factors and sex hormone levels in patients.
    METHODS Patients with polycystic ovary syndrome of kidney deficiency and blood stasis type who visited Xinyu Hospital of Traditional Chinese Medicine (TCM) from May 2022 to July 2023 were included and randomly divided into a control group and an observation group, with 78 patients in each group. The control group received routine Western medicine treatment, while the observation group received Yishen Huayu Zhuyun Formula in addition to the treatment given to the control group. Both groups were treated for 3 menstrual cycles. The improvement of the total TCM syndrome score, uterine spiral artery resistance index (RI), and endometrial thickness on the day of ovulation before and after treatment was compared between the two groups. Pregnancy rates after treatment were also compared between the two groups. Changes in serum dehydroepiandrosterone sulfate (DHEAS), transforming growth factor-β1 (TGF-β1), insulin resistance index (HOMA-IR), interleukin-6 (IL-6), sex hormone-binding globulin (SHBG), and serum luteinizing hormone (LH), follicle-stimulating hormone (FSH), and estrogen (E2) levels before and after treatment were measured in both groups.
    RESULTS After treatment, the total TCM syndrome score and RI decreased in both groups, and the endometrial thickness on the day of ovulation increased, with the observation group showing better results than the control group (P<0.05, P<0.01). Serum DHEAS, TGF-β1, HOMA-IR, and IL-6 decreased in both groups, while SHBG increased, with the observation group superior to the control group (P<0.05, P<0.01). Serum LH and FSH decreased in both groups, while E2 increased, with the observation group outperforming the control group (P<0.05, P<0.01). After treatment, the ovulation rate and pregnancy rate in the observation group were both higher than those in the control group (P<0.05).
    CONCLUSION Yishen Huayu Zhuyun Formula can significantly alleviate the inflammatory state in patients with polycystic ovary syndrome of the kidney deficiency and blood stasis type, regulate sex hormone levels, and has significant clinical efficacy.
