国医大师皮持衡从“脾家实,腐秽当去”辨治糖尿病肾脏病经验

Master of Traditional Chinese Medicine Pi Chiheng’s Experience of Clinical Diagnosis and Treatment of Diabetic Kidney Disease Based on the Principle of “Robust Spleen System Expelling Putrid Waste”

  • 摘要: “脾家实,腐秽当去”源自《伤寒论》,其核心内涵在于脾胃功能充实则腐秽自除。国医大师皮持衡对此经典理论作出创新阐释:一方面从脾胃气化之升降出入、纳运燥湿等生理功能层面进行系统阐发;另一方面从“肠-肾轴”现代机制层面加以科学诠释。同时指出糖尿病肾脏病(DKD)虽病在肾,然脾为关键,提出“脾家不实、阳亏阴损、腐秽积滞”是DKD核心病机,其中脾家不实为病变之肇始,阳亏阴损为进展之关键,腐秽壅滞为终末之结果。临床辨治以“衡通脾胃气化,荡除腐秽壅滞”为基本治法,衍生出衡调枢机复升降、谐循营卫和出入、益火垒土助纳化、畅调水火适燥湿四大辨治思路,形成脾家阴阳理论的特色配伍法则及代表药对,从而为DKD的中医药辨治提供新的思路与参考。

     

    Abstract: “Robust spleen system expelling putrid waste” originates from Treatise on Cold Damage, and its core meaning lies in the fact that when the spleen and stomach functions are robust, putrid waste will naturally be eliminated. Master of Traditional Chinese Medicine Pi Chiheng offers an innovative interpretation of this classic theory: on the one hand, he systematically expounded the physiological functions of the rise and fall, reception and transformation, dryness and dampness of spleen and stomach qi; on the other hand, he provides a scientific explanation from the perspective of the modern “gut-kidney axis” mechanism. He also points out that although diabetic kidney disease (DKD) affects the kidneys, the spleen is crucial. He proposes that “spleen deficiency, yang deficiency and yin depletion, and accumulation of putrid waste” are the core pathogenesis of DKD. Among these, spleen deficiency is the initial stage of the disease, yang deficiency and yin depletion are the key to its progression, and the accumulation of putrid waste is the final result. Clinical diagnosis and treatment are based on the fundamental principle of “balancing and unblocking the spleen and stomach qi transformation, and eliminating putrid waste and stagnation”. This leads to four major diagnostic approaches: balancing and regulating the pivot to restore the rise and fall, harmonizing the circulation of Ying Wei to regulate the entering and leaving, strengthening the fire and earth to aid the reception and transformation, and regulating water and fire to balance dryness and dampness. This has formed the formation of distinctive compatibility rules and representative drug pairs based on the yin-yang theory of the spleen system, providing new ideas and references for the traditional Chinese medicine diagnosis and treatment of DKD.
