基于《内经》“脾藏营”理论辨治代谢综合征

Differentiation and Treatment of Metabolic Syndrome Based on the Theory of “Spleen Storing Nutrients” in the Inner Classic

  • 摘要: 代谢综合征(MetS)患病率持续攀升,严重威胁人类健康。本文将《内经》“脾藏营”诠释为机体代谢稳态调控中枢,解构“脾主运化”为八大环节、四大阶段的动态模型,提出脾胃气化枢机不利为MetS的始动之因,脾营运化障碍、精微输布失常为核心病机,痰湿瘀浊内生、膏脂痰浊沉积为病理结局。MetS病程呈现脾胃气化枢机不利、脾营运化障碍、痰湿瘀浊内生三个阶段,分别对应能量失衡、胰岛素抵抗、多器官损害的现代病理演变。MetS治疗以运脾化浊为核心治法,运脾化浊汤为主方,并根据不同代谢组分精准选用靶药,形成完整诊疗体系。该理论融合经典与现代医学,为MetS中医药治疗提供参考。

     

    Abstract: The prevalence of metabolic syndrome (MetS) continues to rise, posing a serious threat to human health. This paper interprets the Inner Classic concept of “spleen storing nutrients” as the central regulatory hub for metabolic homeostasis and deconstructs the function of “spleen governing transportation and transformation” into a dynamic model comprising eight key links and four major stages. It proposes that “dysfunction of the spleen-stomach qi transformation pivot” is the initiating cause of MetS; that impaired spleen nutrient transportation and transformation, along with disordered distribution of nutrient essences, constitute the core pathogenesis; and that the endogenous generation of phlegm-dampness, stasis, and turbidity, followed by the deposition of adipose-phlegm-turbidity, represents the pathological outcome. The progression of MetS unfolds in three stages—dysfunction of the spleen-stomach qi transformation pivot, impaired spleen nutrient transportation and transformation, and endogenous generation of phlegm-dampness, stasis, and turbidity—which correspond respectively to the modern pathological evolution of energy imbalance, insulin resistance, and multi-organ damage. Treatment centers on the core strategy of "activating the spleen and resolving turbidity", utilizing the Yunpi Huazhuo Decoction as the primary formula, while precisely selecting targeted medications based on specific metabolic components to form a comprehensive diagnostic and therapeutic system. This theoretical framework integrates classical TCM concepts with modern medicine, offering a valuable reference for the TCM treatment of MetS.
