基于《黄帝内经》时空观与络病“三维立体网络系统”论治心力衰竭

Treatment of Heart Failure Based on the Spatiotemporal View of The Yellow Emperor’s Inner Classic and the “Three-Dimensional Network System” of Collateral Disease

  • 摘要: 心力衰竭是多种心血管疾病的终末阶段,具有患病率高、病死率高的临床特点。本文基于《黄帝内经》时空观与络病“三维立体网络系统”理论,系统探析心力衰竭的时空病机演变规律及临床证治思路,认为其发病具有鲜明的时空双重属性。时间层面,以昼夜节律为阴阳消长枢机、四时节律为阳气盛衰根基、病程节律为病势演变纲领,呈现动态病机演变;空间层面,遵循孙络瘀滞为始动环节、大络壅塞为进展核心、络网水停为终末结局的层级病变特点。由此确立“时空通络”核心治则,依时间节律、空间结构用药,发挥调节心络气血津液的综合作用,以期为心力衰竭的中医辨证分期、全周期靶向干预提供理论依据与临床新思路。

     

    Abstract: Heart failure is the terminal stage of various cardiovascular diseases, clinically characterized by high prevalence and high mortality. Based on the spatiotemporal view of The yellow emperor’s inner classic and the “three-dimensional network system” of collateral disease, this paper systematically explores the evolution law of spatiotemporal pathogenesis and clinical syndrome differentiation and treatment ideas of heart failure, and holds that its onset has distinct dual attributes of time and space. In the temporal dimension, the disease presents a dynamic pathogenesis evolution, with the circadian rhythm as the pivot of the growth and decline of yin and yang, the four-season rhythm as the foundation of the vicissitude of yang qi, and the disease course rhythm as the guideline of the progression of disease condition. In the spatial dimension, it follows the hierarchical lesion characteristics that Sunluo stasis is the initial link, large collaterals obstruction is the core of progression, and water retention in the whole collateral network is the terminal outcome. On this basis, the core therapeutic principle of “spatio-temporal unblocking of collaterals” is established. Medication is administered in accordance with temporal rhythm and spatial structure to exert the comprehensive effect of regulating qi, blood and body fluid in the heart collaterals, aiming to provide theoretical basis and new clinical ideas for TCM syndrome differentiation and staging as well as full-cycle targeted intervention of heart failure.
