现代医学背景下原发性肥胖症的中医认识及诊疗刍议

Traditional Chinese Medicine's Understanding and Diagnosis and Treatment of Primary Obesity in the Context of Modern Medicine: A Preliminary Discussion

  • 摘要: 原发性肥胖症是医学界面临的重大难题,现有减重西药并不能满足全部类型肥胖症的治疗需求。中医药治疗原发性肥胖症具有丰富的理论经验和临床实践基础,开发潜力较大。本文在《黄帝内经》“脂人、膏人、肉人”肥胖三型认识的基础上拓展了“小膏人”和“脂膏人”的分型,构建肥胖五型新分类体系,突破了现代医学以单一指标认识肥胖症的局限。并根据原发性肥胖症的异质性特点,从基于能量代谢特征的实壅、虚壅论治,实壅以厚朴三物汤为核心方,根据胃肠饥饿型、情绪饥饿型、脑饥饿型分类辨治;虚壅以六君子汤为核心方,根据低代谢型特点辨治。通过凝练核心病机,归纳核心方药,从而提高中医治疗的精准性,与现有减重西药形成优势互补,提高肥胖症临床疗效。

     

    Abstract: Primary obesity remains a major challenge confronting the medical community, and existing Western weight-loss medications fail to address the therapeutic needs of all obesity subtypes. Traditional Chinese Medicine (TCM) possesses a robust foundation of theoretical insights and clinical practices in the management of primary obesity, exhibiting substantial potential for further development and application. This article expands on the understanding of the three types of obesity—"Zhi Ren, Gao Ren and Rou Ren"—described in The Yellow Emperor's Inner Classic, adding "Xiao Gao Ren" and "Gao-Zhi Ren" types to construct a new five-type classification system for obesity, aiming to overcome the limitations of modern medicine's single-indicator approach to understanding obesity. In light of the heterogeneous nature of primary obesity, this study proposes a treatment strategy based on the pathogenesis of excessive stagnation and deficiency-related stagnation characterized by energy metabolism profiles. For excessive stagnation, Houpo Sanwu Decoction is employed as the core formula, with further differentiation and treatment tailored to three subtypes: gastrointestinal hunger type, emotional hunger type and cerebral hunger type. For deficiency-related stagnation, Liujunzi Decoction serves as the foundational prescription, with syndrome differentiation and treatment customized according to the characteristics of the hypometabolic type. By extracting core pathogenesis and refining core formulas, the precision of TCM treatment is improved, complementing existing Western weight-loss medications and enhancing the clinical efficacy of obesity treatment.
