从“四季脾旺不受邪”探讨儿童反复呼吸道感染间歇期的证治思路

  • 摘要: 赵霞教授基于“四季脾旺不受邪”理论,提出儿童反复呼吸道感染(RRTIs)间歇期的核心病机为脾失健运、肺卫不固,具体表现为脾困乏运,气机不畅;营卫不调,卫外失固;脾困生痰,热积瘀结;脏腑失调,变生他患。其防治要点为运脾防感、补肺固卫,具体治法包括运脾防感,固表止汗;金土同治,调和营卫;清热运脾,消积化痰;调理脏腑,随证加减。该理论为临床防治RRTIs提供了重要的理论依据与诊疗参考。

     

    Abstract: Based on the theory of "thriving spleen in four seasons, preventing pathogenic invasion", Professor Zhao Xia proposes that the core pathogenesis of pediatric recurrent respiratory tract infections (RRTIs) in the intermission stage lies in spleen dysfunction in transportation, unconsolidated lung defense. Specifically, this manifests as spleen deficiency and impaired transportation, leading to stagnation of qi; disharmony of nutritive and defensive qi, resulting in weakened external defense; spleen deficiency generating phlegm, and accumulation of heat and stasis; and dysfunction of zang-fu organs, leading to other complications. The key points of prevention and treatment are promoting spleen transportation to prevent infection and tonifying the lungs to consolidate defensive qi. Specific treatment methods include promoting spleen transportation for infection prevention and consolidating exterior to arrest sweating; treating lung and spleen simultaneously to harmonize nutrient qi and defensive qi; clearing heat and transporting spleen to resolve accumulation and dispel phlegm; regulating the zang-fu organs and performing syndrome-based modification of prescriptions. This theory provides important theoretical basis and clinical guidance for the prevention and treatment of RRTIs.
