Exploration on the Bitterness-Purging and Dispersion-Discharge Methods in the Treatise on Warm-Heat Diseases

Abstract:
This study analyzes the connotation, key points, and clinical application of the bitterness-purging method and the dispersion-discharge method in the Treatise on warm-heat diseases. The bitterness-purging method and the dispersion-discharge method are two treatment methods proposed by the Qing dynasty warm-disease therapist Ye Tian-shi for the gastric distention and fullness caused by damp-heat accumulated in the stomach. The core connotation of the bitterness-purging method is "unblocking and purging with bitter-acrid medicinals, together with using acrid-cool medicinals to promote sweating combined with bitter-cold medicinals to discharge interior heat". In addition, the core connotation of the dispersion-discharge method is "diffusing the exterior with slightly bitter and acrid medicinals". The key to the application of the bitterness-purging method and the dispersion-discharge method is to identify "the damp-heat accumulated in the middle jiao and the blockage of stomach qi". Thus, the corresponding key treatment is "removing pathogenic factors with bitter and acrid medicinals". On this basis, it is critical to correctly identify the differences between the two methods and apply them judiciously. The bitterness-purging method and the dispersion-discharge method can be applied in the treatment of chronic gastritis, chronic obstructive pulmonary disease, tuberculous pleurisy, sicca syndrome, as well as novel coronavirus infection.
Exploring the Application of the Four-Manifestation Constitution Theory of the Spiritual Pivot in the Identification and Treatment of Stroke Based on the Hetu Theory

Abstract:
Based on the four-manifestation constitution theory of the Hetu theory, combined with the Taiyang, Shaoyang, Taiyin, Shaoyin system in the Spiritual pivot, the constitution of human beings is classified into four categories: Taiyang, Shaoyang, Taiyin, Shaoyin. The above classification follows the physiological and pathological characteristics of human beings with yin and yang as the base. Stroke is a common critical illness in clinical practice with such problems as limited treatments and high recurrence rate, which are closely related to the physical characteristics of patients. By analyzing the characteristics of the four-manifestation constitution and yin-yang proportion, the study explores the relationship between the four-manifestation constitution and stroke, and proposes a way to treat stroke by regulating yin and yang, which provided new ideas and methods for clinical treatment of stroke.
Effect of Yiqi Huoxue Formula on Myocardial Fibrosis in Patients with Heart Failure after Acute ST-Segment Elevation Myocardial Infarction

Abstract:
  OBJECTIVE  To evaluate the effect of Yiqi Huoxue formula on myocardial fibrosis in patients with heart failure after acute ST-segment elevation myocardial infarction.  METHODS  A total of 41 patients with acute ST-segment elevation myocardial infarction with qi deficiency and blood stasis who attended the Jiangsu Integrated Chinese and Western Medicine Hospital Affiliated to Nanjing University of Chinese from February 2019 to January 2021 were selected and divided into 20 cases in the control group and 21 cases in the study group according to the random number table. The control group was given standard treatment for myocardial infarction, while the study group was treated with Yiqi Huoxue formula based on the standard treatment. The course of treatment was 12 weeks. Before and after treatment, we observed the changes of both groups in the traditional Chinese medicine (TCM) syndrome score, clinical efficacy, human plasma lipoprotein-related phospholipase A2 (Lp-PLA2), hypersensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), N-terminal pro-brain natriuretic peptide (NT-proBNP), 6 minutes walk test (6MWT), echocardiographic parameters [left ventricular ejection fraction (LVEF), left atrium dimension (LAD), left ventricular end-diastolic dimension (LVEDD)], extracellular volume (ECV) and longitudinal relaxation time (T1) by cardiac magnetic resonance imaging.  RESULTS  After treatment, the TCM syndrome scores were reduced in both groups (P < 0.05, P < 0.01), but study group presented better curative effects in chest tingling, shortness of breath, faint low voice, lusterless complexion, post-activity fatigue, spontaneous sweating, scaly skin, as well as total score (P < 0.05, P < 0.01). In addition, the clinical efficacy of TCM in the study group was better than that in the control group (P < 0.05). The levels of Lp-PLA2, hs-CRP, IL-6, NT-proBNP, ECV and T1 were reduced in both groups (P < 0.05), but the results of the study group were better than those of the control group (P < 0.05). Besides, the results of 6MWT and LVEF were increased in both groups (P < 0.05), but the results of the study group were better than those of the control group (P < 0.05). There was no statistically significant difference in the comparison of major cardiovascular adverse events between the two groups (P>0.05).  CONCLUSION  Yiqi Huoxue formula can mitigate the clinical symptoms of patients with acute ST-segment elevation myocardial infarction after percutaneous coronary intervention (PCI), improve the clinical efficacy of traditional Chinese medicine, and inhibit myocardial fibrosis. The mechanism may be related to the reduction of the inflammatory response.
Effect of Kangxinshuai Formula on Cardiac Function and Oxidative Stress in Patients with Chronic Heart Failure

Abstract:
  OBJECTIVE  To observe the effect of Kangxinshuai formula on cardiac function and oxidative stress in patients suffering from chronic heart failure with qi-yin deficiency and phlegm-stasis combined with water-stagnation (chronic heart failure for short).  METHODS  A total of 100 patients with chronic heart failure who met the inclusion criteria were selected and divided into 50 cases each in the observation group and the control group according to the random number table. The control group was given standardized treatment for chronic heart failure, while the observation group was treated with the Kangxinshuai formula on top of the treatment of the control group. The course of treatment was 12 weeks. Before and after the treatment, we observed the changes in the traditional Chinese medicine (TCM) syndrome score, clinical efficacy, New York Heart Association (NYHA) cardiac function classification, 6 min walking test (6MWT), Minnesota heart failure quality of life questionnaire (MLHFQ), echocardiographic parameters [left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular ejection fraction (LVEF)], serum N-terminal pro brain natriuretic peptide (NT-proBNP), soluble growth stimulator gene 2 protein (sST2), as well as endogenous antioxidant stress factors [heme oxygenase-1 (HO-1), peroxidase (SOD), glutathione peroxidase (GPx), quinone oxidoreductase-1 (NQO-1)], and evaluated the related safety indexes.  RESULTS  After treatment, the TCM syndrome score, clinical efficacy, NYHA cardiac function classification, 6MWT, MLHFQ score, echocardiographic parameters, NT-proBNP, and sST2 showed better results in the observation group (P < 0.05), so the clinical effects of the observation group were better than those of the control group (P < 0.05, P < 0.01). Besides, endogenous anti-oxidative stress factor levels were improved in both groups (P < 0.01), while the observation group performed better than the control group (P < 0.01).  CONCLUSION  Kangxinshuai formula combined with standardized treatment for chronic heart failure can improve the clinical efficacy, cardiac function classification and prognosis of chronic heart failure. The possible mechanism is related to increasing the level of endogenous anti-oxidative stress factors, reducing cardiac oxidative stress damage, improving antioxidant capacity, and delaying cardiac remodeling.
Effect of Gubu Decoction Combined with Compound Huangbai Liquid on the Improvement of Wound in the Post-Interventional Operation for Dry Gangrene of Diabetic Foot

Abstract:
  OBJECTIVE  To observe the effects of Gubu decoction combined with compound Huangbai liquid on wound repair, granulation tissue growth and microvascular circulation in the foot after intervention operation for dry gangrene of diabetic foot.  METHODS  A total of 80 patients with dry gangrene of diabetic foot were selected and grouped by random number table, 40 patients in the control group and 40 patients in the treatment group. The control group was treated with compound Huangbai liquid after vascular intervention operation, while the treatment group was given with Gubu decoction combined with compound Huangbai liquid. The treatment course was four weeks. Before and after the treatment, we observed and compared the changes of vascular endothelial growth factor (VEGF), transforming growth factor β1 (TGF-β1), basic fibroblast growth factor (bFGF), nitric oxide (NO), endothelin (ET-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), interleukin-13 (IL-13), tumor necrosis factor (TNF-α), fibrinogen level, the microvascular blood perfusion in the foot, NRS score, granulation tissue growth, foot wound repairment, traditional Chinese medicine (TCM) syndrome scores, as well as the clinical efficacy of the two groups.  RESULTS  After treatment, microvascular blood perfusion in the foot increased in both groups (P < 0.01), but the result in the treatment group was better than that in the control group (P < 0.01). Besides, the area of wound, TCM syndrome scores and NRS score were reduced in both groups compared with those before treatment (P < 0.01). The patients in treatment group were fewer compared with the control group (P < 0.01), but the granulation tissue growth and wound repairment in the treatment group were better than in the control group (P < 0.01). The total efficiency of the treatment group was higher than that of the control group (P < 0.05). The levels of TNF-α, sVCAM-1 and IL-13 in both groups were lower than before treatment (P < 0. 01), but the treatment group showed better results (P < 0.01). The levels of fibrinogen and ET-1 in both groups were lower than before treatment (P < 0.01), but the treatment group showed better results (P < 0.01). The levels of NO were higher than before treatment (P < 0.01), but the result in the treatment group was higher than that in the control group (P < 0.05). The levels of VEGF, TGFβ1 and bFGFβ1 in both groups were higher than the control group (P < 0.05). The levels of VEGF, TGFβ1 and bFGF in the two groups were higher than before treatment (P < 0.01), but the levels in the treatment group were higher than those in the control group (P < 0.01).  CONCLUSION  The combination of Gubu decoction and Huangbai liquid in the post-interventional operation for dry gangrene of diabetic foot can enhance the level of growth factors, suppress inflammation, improve microvascular circulation and endothelial function of the foot, relieve pain, promote granulation tissue growth and wound repairment, and improve clinical efficacy.
Effect of Qufeng Zhike Decoction on TRPV1 Pathway in Guinea Pig Model with Increased Cough Sensitivity

Abstract:
  OBJECTIVE  To observe the effect of Qufeng Zhike Decoction on the expression of the TRPV1 pathway in a guinea pig model with increased cough sensitivity, and to explore its mechanism of intervention on the laryngeal cough.  METHODS  The laryngeal cough (guinea pig with increased cough sensitivity) model was prepared by intraperitoneal injection of egg protein sensitization combined with smoking and randomly divided into the blank group, model group, agonist group (Qufeng Zhike Decoction+capsaicin), Chinese medicine group (Qufeng Zhike Formula) and control group (Meimin Pseudoephedrine Solution). The changes in cough times before and after treatment were observed in each group. The morphology of throat mucosa, tracheal mucosa, and lung tissue was observed by H&E staining. The expressions of serum interleukin (IL)-1β, IL-10, and IgE were detected by ELISA. The expression levels of TRPV1, CGRP, NKA, and SP in the throat and tracheal mucosa were also detected by ELISA. The expressions of IL-1β, TRPV1, GFAP, and NGF in the cough center of medulla oblongata were detected by Western blot.  RESULTS  Compared with the blank control group, the cough sensitivity of the model group was significantly increased (P < 0.01). Submucosal edema, inflammatory cell infiltration, and eosinophil count were observed in the throat, tracheal mucosa, and lung tissue. The expressions of serum IL-1β and IgE were increased (P < 0.01), IL-10 was decreased (P < 0.01), the contents of TRPV1, NKA, CGRP, and SP in the throat and tracheal mucosa were increased (P < 0.01), and the protein expressions of IL-1β, TRPV1, GFAP and NGF in the medullary cough center were increased (P < 0.01). Compared with the model group, the Qufeng Zhike Decoction had significantly reduced cough times (P < 0.01), significantly improved submucosal edema and inflammatory cell infiltration, significantly decreased serum eosinophil count and eosinophil percentage (P < 0.05, P < 0.01), significantly decreased serum IL-1β and IgE expressions (P < 0.01), and significantly increased IL-10 expression (P < 0.01). The contents of TRPV1, NKA, CGRP, and SP in the throat and tracheal mucosa were decreased (P < 0.05, P < 0.01), and the protein expressions of IL-1β, TRPV1, GFAP, and NGF in the cough center of medulla oblongata were decreased (P < 0.01). After treatment with Qufeng Zhike Decoction, compared with the Meimin Pseudoephedrine Solution, there were significant differences in serum IgE content, CGRP content in tracheal mucosa, and protein expressions of TRPV1, GFAP and NGF in cough center of the medulla oblongata (P < 0.05, P < 0.01), but no other significant differences; compared with the Qufeng Zhike Decoction+capsaicin, the difference between the two groups was statistically significant (P < 0.05, P < 0.01).  CONCLUSION  Qufeng Zhike Decoction in treating laryngeal cough may regulate the expression of inflammatory factors through TRPV 1 pathway, inhibit neurogenic airway inflammation, reduce the high sensitivity of the peripheral airway and cough center, and reduce the recurrence.
Huangqi-Danshen in the Regulation of ASIC1a/CaMKⅡδ Signaling to Inhibit Cardiomyocytes from Pyroptosis in Acidic Microenvironment

Abstract:
  OBJECTIVE  To observe the protective effect of Huangqi-Danshen on cardiomyocytes pyroptosis or injury in acidic microenvironment induced by lactic acid and the regulatory mechanism based on the acid-sensitive ion channel 1a (ASIC1a) or calcium/calmodulin-dependent protein kinase Ⅱδ (Ca MKⅡδ) signaling pathway.  METHODS  Lactate was used to stimulate H9C2 cardiomyocytes to establish the cell injury model in the acidic environment of pH 6.5. The rats were divided into the normal control group, model group, Huangqi-Danshen extract (HQ) group, HQ + ASIC agonist Nocistatin group, as well as ASIC inhibitor NS383 positive drug control group. The viability of cardiomyocytes was measured by CCK-8 method. The apoptosis of cardiomyocytes was detected by flow cytometry. Besides, the expression of ASIC1a was detected by immunofluorescence. The mRNA expression of NLRP3, Caspase-1, IL-1β, and GSDMD was analyzed by qPCR. In addition, the protein expression of ASIC1a, CaMKⅡδ, NLRP3, Caspase-1, Cleaved Caspase-1, GSDMD, and GSDME-N was detected by Western blot.  RESULTS  In the acidic environment of pH 6.5, the viability of cardiomyocytes was significantly reduced (P < 0.01), but the apoptosis was significantly increased (P < 0.01). The mRNA expressions of NLRP3, Caspase-1, IL-1β and GSDMD were significantly increased (P < 0.01), and the protein expressions of ASIC1a, CaMKⅡδ, NLRP3, Caspase-1, Cleaved Caspase-1, GSDMD and GSDME-N were also significantly increased (P < 0.01). In addition, we found that HQ (60 μg·mL-1) could significantly promote the viability of cardiomyocytes in acidic microenvironment (P < 0.01), inhibit cardiomyocyte apoptosis (P < 0.01), and down-regulate the mRNA expressions (NLRP3, Caspase-1, IL-1β, GSDMD) combined with protein expressions (ASIC1a, CaMKⅡδ, NLRP3, Caspase-1, Cleaved Caspase-1, GSDMD, GSDME-N) (P < 0.01). Besides, the above effects of HQ can be reversed by ASIC agonist Nocistatin.  CONCLUSION  Huangqi-Danshen can down-regulate ASIC1a/CaMKⅡδ signaling to inhibit NLRP3 inflammasome -mediated cardiomyocyte pyroptosis in acidic microenvironment.
Analysis of the Volatile Components of Different Parts of Acanthopanax Senticosus by HS-SPME-GC-MS Combined with Chemometric Methods

Abstract:
  OBJECTIVE  To conduct a comparative analysis of the volatile components of different parts of Acanthopanax senticosus, and provide scientific reference for their subsequent clinical application and development.  METHODS  Headspace solid-phase microextraction coupled with gas chromatography-mass spectrometry (HS-SPME-GC-MS) was used to extract and analyze the volatile components of the samples of various parts of Acanthopanax senticosus. The relative contents of the components were determined and compared by the peak area normalization method, and the differences of the components were analyzed with principal component analysis (PCA) and hierarchical cluster analysis (HCA).  RESULTS  A total of 167 volatile components were identified from samples of various parts of Acanthopanax senticosus, with 71, 61, 59, 48 and 54 volatile components identified in the root, velamen, stem, leaf and fruit, respectively, mainly as alkenes and alcohols. 10 common components were identified in five parts, including terpinolene, β-staphylene and α-rutinolene, etc. The results of HCA analysis clustered Acanthopanax senticosus root and fruit into one group, velamen and leaf into one group, and stem into a separate group. The PCA scores in the order were: root, stem, fruit, leaf and bark.  CONCLUSION  The types and contents of volatile components in the various parts of Acanthopanax senticosus are different, providing a scientific basis for the further development and utilization of the unconventional medicinal parts of Acanthopanax senticosus for medicinal and food purposes.
Study on Processe of Aristolochia Contorta Bge. Based on Characteristic Spectrum Combined with Effective Toxicity Value and Appearance Characters

Abstract:
  OBJECTIVE  Based on the characteristic spectrum combined with the determination of effective Toxiciy appearance character, to study the changes of toxic components in the processing of Aristolochia contorta Bge.  METHODS  The characteristic chromatogram of 17 batches of Aristolochia contorta Bge. was established by HPLC, and the parameters of honey, dilution times, humid time and frying time were investigated. Samples were taken at intervals in the process, the chromaticity value and characteristic map were measured, and the comprehensive evaluation of appearance characters and intrinsic toxicity values was carried out.  RESULTS  10 common peaks were calibrated and 6 characteristic peaks were identified in the characteristic spectrum of Aristolochia contorta Bge., among which peak 2 was magnoflorine, peak 5 was 7-hydroxy aristolochic acid A, peak 6 was aristolochic acid D, peak 8 was aristolochic acid B, peak 9 was aristolochic acidⅠ, and peak 10 was aristolochic acidⅠ. According to the pre-treatment investigation, the dosage of honey was 25 kg of refined honey per 100 kg of products to be roasted, the honey was diluted by 3 times amount of water, and the humid time was 60 min. The processing process showed that the peak area of the toxic components of Aristolochia contorta Bge. decreased obviously with the processing time, and the toxic value of magnoflorine and five toxic components gradually increased. According to the comprehensive evaluation of appearance traits and intrinsic toxicity value, the samples processed for 19-27 min ranked in the top five, processing time was determined to be 23-27 min.  CONCLUSION  After Aristolochia contorta Bge. is processed, the relative contents of toxic components decreases, while the toxic value increases. The quality evaluation method of effective toxicity value combined with appearance characteristics can provide reference for the processing of toxic Chinese herbal pieces
Study on Core Prescriptions and Drugs of TCM Against Antituberculosis Drug-Induced Liver Injury Based on Data Mining and Efficacy Screening

Abstract:
  OBJECTIVE  Based on data mining and zebrafish model, to analyze the medication rules of TCM on antituberculosis drug-induced liver injury (ATDILI), and screen the core prescriptions and drugs.  METHODS  Literature was collected, prescriptions database was established by TCM inheritance system for data mining, and efficacy of core prescriptions and drugs were verified in zebrafish model.  RESULTS  A total of 342 prescriptions were screened, and 41 common drugs, 17 common drug pairs, and one core prescription were obtained. In the ATDILI model, the malformation rate and mortality rate of juvenile zebrafish were significantly reduced(P < 0.05), the morphology was improved, and the fluorescence area and intensity of liver increased significantly under the core prescriptions and drugs(P < 0.01).  CONCLUSION  TCM intervention in ATDILI is based on the general principle of tonifying deficiency, clearing heat and dampness, focusing on the simultaneous treatment of liver and spleen. The core prescriptions and drugs obtained from data mining showed liver protection effect in the ATDILI model of zebrafish, which verified the reliability and applicability of data mining technology. In this study, data mining and pharmacological experimental validation were combined to provide reference for the treatment of ATDILI.
Meta-Analysis of Clinical Efficacy and Safety of Xijiao Dihuang Decoction Combined with Western Medicine in the Treatment of Anaphylactoid Purpura

Abstract:
  OBJECTIVE  To systematically evaluate the efficacy and safety of Xijiao Dihuang decoction combined with western medicine in the treatment of anaphylactoid purpura.  METHODS  A comprehensive search of PubMed, CBM, CNKI and other databases was conducted to collect randomized controlled trials of Xijiao Dihuang decoction combined with western medicine in the treatment of anaphylactoid purpura. The literature was screened, information extracted and quality evaluated according to inclusion and exclusion criteria. The outcome indicators were evaluated according to the GRADE evidence quality grading system, and the risk of bias was assessed and Meta-analysis was performed using RevMan5. 3 software.  RESULTS  A total of 23 studies involving 1, 919 patients were included. Meta-analysis showed that compared with the simple western medicine treatment, the integrated Chinese and western medicine combined with Xijiao Dihuang decoction could improve the total effective rate[RD=0.18, 95%CI(0.15, 0.21), P < 0.01], reduce the recurrence rate [RD=-0.17, 95%CI(-0.334 5, -0.004 2), P < 0.05] and the adverse reaction rate [RD=-0.05, 95%CI(-0.090 5, -0.004 7), P < 0.01]. Besides, it could also reduce purpura regression time [MD=-6.73, 95%CI(-10.58, -2.89), P < 0.01], rash regression time [MD=-3.08, 95%CI(-4.25, -1.92), P < 0.01], gastrointestinal symptoms regression time [MD=-3.49, 95%CI(-5.54, -1.43), P < 0.01], abdominal pain regression time [MD=-1.97, 95%CI(-2.23, -1.71), P < 0.01], as well as joint swelling and pain regression time [MD=-6.60, 95%CI(-8.88, -4.31), P < 0.01]. The GRADE of outcome indicators included medium, low and extreme low levels.  CONCLUSION  Xijiao Dihuang decoction combined with conventional western medicine can improve the total effective rate of anaphylactoid purpura. In addition, it can reduce the recurrence rate, and the regression time of all clinical symptoms without increasing the adverse reaction rate.

Abstract:
Damp-heat syndrome of large intestine is the core syndrome of ulcerative colitis disease (UC).In this paper, we summarized the research status of TCM syndrome characteristics, large intestine damp heat syndrome characteristics and the related biological basis of ulcerative colitis in recent years, it was confirmed that the syndrome of damp-heat in large intestine was the most common TCM syndrome in the ulcerative colitis, but the existing syndrome diagnostic criteria were still subjective, the objective physical and chemical indicators, the biological basis of syndrome research is lack of effective integration, and no specific syndrome markers have been found. In the future, it is necessary to strengthen the combination of multi-subjects, pay attention to the combination of disease-syndrome-symptom, look for specific diagnostic markers of damp-heat syndrome of large intestine in ulcerative colitis, and establish the subjective and objective diagnostic standards of the combination of disease and syndrome, to perfect the scientific theoretical system of TCM syndrome.

Abstract:
Exosomal lncRNAs play an important role in the occurrence and development of colorectal cancer, and can affect the process of colorectal cancer by inducing immune escape, driving epithelial-mesenchymal transition, increasing resistance to chemotherapy and targeted therapy, and promoting angiogenesis. Ancient and modern doctors believe that the basic pathogenesis of colorectal cancer is based on the deficiency of the standard of the real, the original deficiency is the deficiency of righteous qi, and the standard of the real is caused by the interaction of pathological factors such as phlegm, dampness, heat, toxin, and blood stasis. The occurrence and development of rectal cancer are highly consistent with the understanding of the pathogenesis of colorectal cancer in traditional Chinese medicine. This article combs the mechanism of exosomal lncRNAs in the occurrence and development of colorectal cancer in recent years and the research of traditional Chinese medicine intervention, and provides ideas for future research on the prevention and treatment of colorectal cancer with integrated traditional Chinese and western medicine.
