SHI Xin-de
2018, 34(5): 433-435.
Abstract:
From the aspects of drug composition, disease syndrome, etiology, pathogenesis and action sites, the prescriptions of Fuling (Poria) and Baizhu (Rhizoma atractylodis macrocephalae) (referred to as Lingzhu Formula) in Treatise on Cold Damage and Essentials from the Golden Cabinet were compared and combed. It is believed that the main treatment of Lingzhu Formula is a type of disease syndrome with abnormal water and liquid metabolism. According to the different positions of fluid retention, the composition of Lingzhu Formula is also different: If the fluid retention accumulated in the head, use Guizhi (Ramulus cinnamomi) Decoction removes Guizhi and add Fuling Baizhu Decoction. If the fluid retention accumulated in the epigastic, chest and rib-side, then use Linggui Zhugan Decotion, Fuling Zexie Decoction. If the fluid retention accumulated in the diaphragm, use Zhuling Powder; in the waist and abdomen, use Ganjiang Lingzhu Decoction; in the exterior and interior, use Wuling Powder. In addition, if the patient with yang deficiency and cold syndrome, it can use Zhenwu Decoction, Fuzi Decoction; with qi deficiency, use Lizhong and Fuling Decoction; with blood stasis, use Danggui Shaoyao Powder.
Reflections on Classification of TCM Treatment Methods
HUANG Yu-yan, ZHANG Li-ping, TANG Er-qun
2018, 34(5): 436-439.
Abstract:
There is no comprehensive strategy for classification of treatment methods, even the most classical classification: CHENG Guopeng's eight methods. The determination of treatment methods is a subjective speculation process. Treatment methods could be expressed in various ways, so the classification of treatment methods has the properties of subjectivity, limitation and multi-dimension. Starting with the TCM theoretical system, accompanied by related pathogenesis and etiology, it is more feasible to categorize the treatment methods. To solve the cross-repetition and tedious problem, treatment methods can be screened by determining the criteria concept and synonyms, selecting the smallest unit, and screening out the efficacy.

SONGDe-yin, SHISuo-fang
2018, 34(5): 440-442.
Abstract:
Combining with the viewpoints of the past doctors, this paper discusses the addition and subtraction of Xiaoqinglonng Decoction in the Treatise on Febrile Diseases, which "Removing Ephedra for asthma" is "in accordance with the meaning of ZHANG Zhongjing". And combing the relevant provisions in the Essentials from the Golden Cabinet to carry out a theoretical analysis and believes that it is excessively pursued and has no practical significance. According to Zhongjing's spirit of differentiation and treatment syndrome, Xiaoqinglong Decoction needs to distinguish between healthy qi and pathogenic qi relationship in specific clinical application. It can be used for patients with cold and good constitution, suitable for sweat method; if the patients with fluid retention and asthma, mild cold-exterior or accompanied with deficiency syndrome, it should not be used.

2018, 34(5): 474-479.
Abstract:
OBJECTIVE To evaluate and establish a method for rapid comparison of transdermal penetration enhancement effect of essential oils (EOs) from Chinese materia medica (CMM). METHODS  The active components (ferulic acid, albiflorin, paeoniflorin, tetrahydrocolumbamine, tetrahydropalmatine) in Xiangfu Siwu Decotion (XFSWD)were used as model drugs for transdermal permeation study. The oil/water partition coefficient values of the above active components were measured. Six EOs of interior-warming drugs were used as penetration enhancer (PE). The Franz diffusion cell method was applied to investigate the effects of EOs on transdermal permeation of active components in XFSWD. The impact of EOs on skin integrity was evaluated by electrical resistance (ER) of the skin. Finally, the correlation between the results of skin resistance dynamics and in vitro transdermal permeation experiment was assessed. RESULTS The oil-water partition coefficients of ferulic acid, albiflorin, paeoniflorin, tetrahydrocolumbamine, tetrahydropalmatine were measured to be -0.93,-0.59,-0.38,1.97, 2.15. Six EOs of interior-warming drugs as PE could significantly enhance the permeation of active components in XFSWD,especially the polar components. The EOs with hot nature possess better penetration ability than that of warm nature. Besides, there was a good correlation between the results of skin resistance dynamics and in vitro transdermal permeation experiments for above drugs with different polarity(r=0.748 1-0.812 9). CONCLUSION The penetration enhancing effect of EOs of interior-warming drugs are affected by drug four nature property characteristics of CMM.The ER method can effectively identify the penetration enhancing effect of EOs.

2018, 34(5): 491-494.
Abstract:
OBJECTIVE To study the anti-inflammatory effect of tripterine on bovine type Ⅱ collagen-induced arthritis in mice and its mechanism of action on T cells. METHODS DBA/1J mice were used to induce CIA model induced by bovine type Ⅱ collagen. After the model was established, tripterine of 2 mg/kg was given to the mice and the joint score was recorded. The mice were sacrificed after 70 d of secondary immunization. The levels of IL-17, IL-6 and TGF-β in the plasma of mice were detected by ELISA. Pathological changes of the joints were observed by HE staining. Flow cytometry was used to detect the ratio of Th17 to Treg in mouse spleen cells. RESULTS The joint scores of tripterine in mice were significantly lower than that in model group (P<0.05). HE staining showed that tripterine improved the synovitis infiltration (P<0.05); ELISA showed that tripterine significantly decreased the levels of Th17-associated cytokine IL-6 (P<0.01). Flow cytometry showed that tripterine increased the proportion of Treg cells (P<0.05) while significantly decreased the proportion of Th17 cells in the spleen of mice (P<0.01). CONCLUSION  Tripthenin can effectively alleviate the symptoms of joint inflammation in CIA mice, inhibit synovitis inflammatory cell infiltration, angiogenesis and cartilage destruction. Tripterine treatment of CIA mice inflammation may be through the reduction of IL-6 secretion in CIA mice with anti-inflammatory cytokines involved in the immune balance regulation process.

YUYun, SHILe, YUBin, BIANYong
2018, 34(5): 499-503.
Abstract:
OBJECTIVE To investigate the effects of geniposide on Th17/Treg balance and local inflammatory factors in rats with rheumatoid arthritis. METHODS We established the CIA model to observe the histopathological changes of ankle joints in rats. ELISA was used to detect plasma TNF-α, and Western blot was used to detect the protein contents of transcription factor Foxp3 and RORγt. The expression of IL-10 and IL-17 in joint synovial tissue was detected by immunohistochemistry. RESULTS Compared with the normal control group, the serum concentration of TNF-α, IL-17 and ROR-γt and the expression of IL-17 in joint synovial tissue were significantly higher in the model group (P<0.01), while the concentration of IL-10 and Foxp3 and the expression of IL-10 in joint synovial tissue decreased significantly in the model group (P<0.01). Compared with the model group, the serum concentration of TNF-α, IL-17 and ROR-γt and the expression of IL-17 in joint synovial tissue were significantly lower in the middle- and high-dose groups of geniposide (P<0.05, P<0.01), while the concentration of IL-10 and Foxp3 and the expression of IL-10 in joint synovial tissue increased significantly in the middle- and high-dose groups of geniposide (P<0.05, P<0.01). CONCLUSION There was an imbalanced Treg/Th17 differentiation in the joint synovium of CIA rats. By inhibiting the differentiation of Th17 cells in joint synovium, geniposide might induce the generation of Treg cells and then restore the Treg/Th17 balance in the diseased area.

The medium dose group improved cell morphology, reduced apoptosis and the expression of NF-κB, P53, caspase-3 and P21 proteins. CONCLUSION By promoting cell proliferation, inhibiting cell apoptosis, increasing the secretion of NO and the activity of TNOS and iNOS, reducing the expression of NF-κB, P53, caspase-3 and P21 signaling pathway proteins, thus protecting vascular endothelial cells is the mechanisms for the treatment of hemorrhagic stroke.
Experience of Nephropathy by National Chinese Medicine Master ZOU Yanqin of "Fom up Treat down Disease" in Clearing Throat

2018, 34(5): 513-515.
Abstract:
The throat is closely related to the kidney. National Chinese Medicine of ZOU Yanqin has created the theory of treating nephropathy with "from up treat down disease" in the treatment according to pattern differentiation, and pays attention to clearing throat at the same time. The symptom of throat at acute stage are given to disperse wind and discharge heat, clear heat from throat to dispel evils, and treat the branch and prevent evil heat transmission to kidney. Supplementing and boosting the lung and kidney, nourishing yin and clearing throat at remission stage to support right and treat the root.

2018, 34(5): 516-519.
Abstract:
The wind-cold-dampness arthralgia syndrome is quite common in orthopedics. It believes that the external cause of the syndrome is intermingled of pathogens evils, and the internal cause is morbid Yang qi and evil lies in the body meridians. Morbid Yang qi of spleen and kidney is especially important in the pathogenesis of the disease. The treatment should be based on warmth, which can warm spleen yang, resolve constraint and calm the mind, warm kidney yang and nourish kidney yin. The meridian block leads to disharmony of qi and blood and malnutrition sinew and bone, and dredging meridian is also an important treatment. The dredging therapy can be based on the specific syndrome and its pathogenesis, or dispel pathogen or reinforce healthy qi, or combined or focus on one side. Warming meridian and dredging collateral in treating wind-cold-dampness arthralgia syndrome of orthopedics, should consider the individual differences in age, physical constitution, disease course, disease position, etc. The prescriptionis certified by the symptom, and the medicine is compatible with the symptom.

2018, 34(5): 520-523.
Abstract:
This paper summarizes professor SHAN Zhaowei's experience in treating chronic diarrhea. He believes that the spleen deficiency and exuberant phlegm is the root, while dampness-heat, static blood and accumulated food is the branch. The disease is in the spleen and stomach, involving in liver and kidney. According to the characteristics of diarrhea to identify the asthenia and sthenia, cold and heat, viscera of spleen, liver, kidney and flexible combination of prescriptions. Meanwhile professer SHAN is good at treating chronic diarrhea by using wind medicine and combining warm medicine with cold medicine. When taking care of other pathological factors, it is taken into account the methods of digestion, invigorating blood and resolving stasis. Pay attention to tongue diagnosis and tongue syndrome.
Progress on Effective Components of Traditional Chinese Medicine Indiabetic Nephropathy
WANG Ling, LI Yu, GU Chun-yan
2018, 34(5): 524-528.
Abstract:
Diabetic nephropathy (DN), as a serious complication of type 2 diabetes mellitus (T2DM), is the major cause of end-stage renal disease with high mortality worldwide. The progressive kidney disorder is pathologically characterized by glomerular fibrosis, thickened glomerular and tubular basement membranes, mesangial cells expansion, accumulation of the extracellular matrix and increased mesangial hypertrophy. Although the exact causative factor of DN remains unclear, several mechanisms have been postulated, such as oxidative stress, increased inflammation, renal hyperfiltration and renal injury. Therefore, lots of synthesized drugs were developed to focus on these therapeutic targets to cure DN. However, clinical severe side effects have emerged following the treatments. Traditional Chinese medicine has showed the advantages to remedy DN for its natural and safe character. This review will highlight the classic and effective components of traditional Chinese medicine to cure diabetic nephropathy in preclinical researches to offer new therapeutic strategies for an adequate pharmacotherapy of DN.
