2016, 32(6): 501-505.
Abstract:
Knee osteoarthritis(KOA) is a common disease in the elderly. Pain, the most common symptom of KOA, is seen in more than 65% of the patients as main manifestation in clinic. At present, there is no clear pathogenesis and specific treatment of KOA. Conservative treatment, mainly including the control of inflammation and delay of cartilage degeneration, is for controlling pain and other clinical symptoms. KOA belongs to "Bone Bi" and "Bi syndrome" of traditional Chinese medicine. By reviewing the literature on the treatment experience of KOA in orthopedics and traumatology of TCM and combining the experience of department of orthopedics and traumatology in our hospital, diagnostic and therapeutic ideology centered by the control of intractable pain of KOA is put forward with the therapeutic system guided by the concept of truncating and reversing and the combination of cold and heat.

SUN Zheng, WU Jiu-long, HU Guang-yong, LU Meng-jiang, ZHANG Jian-bin
2016, 32(6): 506-508.
Abstract:
By exploring the discussions about the nine-needle in Neijing and making a summary of its number shapes and applications, we find that the nine-needle theory has the feature of concerning morphological structure which is the same as new acupuncture tools and therapies today and very different from "mind-balance" of filiform needle. However, we consider that the nine-needle theory which possesses the feature of "structure-needling" should but did not have enough influence on the development of acupuncture. So we call for paying more academic attention to "physics" during the development of modern acupuncture and making acupuncture more practical.

Abstract:
OBJECTIVE To explore the correlation between polymorphisms in the toll-like receptor 4 (rs1927914, rs10759932 and rs11536889) and susceptivity, gene expression, cytokines and blood lipid metabolism in southern Han Chinese with stroke due to wind-phlegm obstruction, qi deficiency and blood stasis. METHODS 559 stroke cases were divided into two groups based on case-control study method, with 248 cases in the qi deficiency and blood stasis group, 311 cases in wind-phlegm obstruction group. In addition, another 605 cases were involved as the healthy control group with the corresponding gender and age. All the subjects were from the Han nationality. Sequenom MassARRAY technology was adopted for testing and genotyping of rs1927914, rs10759932 and rs11536889 in the toll-like receptor 4. Real-time fluorescence quantification(qPCR) was applied for examine mRNA expression. Enzyme linked immunosorbent assay(ELISA) was used to detect the level of interleukin-1β(IL-1β), interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-12(IL-12) and tumor necrosis factor-α(TNF-α). HITACHI 7600 automatic biochemical analyzer was applied to test the total cholesterol(TC), triglyceride(TG), high density lipoprotein cholesterol(HDL) and low density lipoprotein cholesterol(LDL). RESULTS There existed a statistical difference between the male cases of the qi deficiency and blood stasis group and male cases of the control group in the distribution of gene frequency of rs10759932(P<0.05). The locus polymorphism of both rs1927914 (dominant model: OR=0.65, 95%CI=0.43-0.96, Padj=0.031) and rs10759932 (dominant model: OR=0.64, 95%CI=0.41-0.99, Padj=0.046) were closely related with susceptivity of stroke male of qi deficiency and blood stasis type. The IL-8 level of male stroke patients of qi deficiency and blood stasis type with the rs10759932 TT genotype was much lower than those with CC+CT(P<0.01). Rs11536889 polymorphism was notablely related to TG level(additive model: Padj=0.021, dominant model: Padj=0.013) and HDL level(dominant model: Padj=0.032) in stroke patients of qi deficiency and blood stasis type. CONCLUSION TLR4 polymorphisms exert certain influence on the occurance of stroke due to qi deficiency and blood stasis in Han Chinese men and they have been involved in the inflammatory reaction and blood lipid metabolism.

Abstract:
OBJECTIVE To observe the efficiency of the method of soothing liver, calming nerves, descending adverse qi and protecting membrane in treating patients with non-erosive reflux gastroesophageal disease(NERD) and its role in reducing the expression of CGRP and SP. METHODS 37 patients were randomly divided into treatment group(20 cases) and control group (17 cases). Patients in the control group were treated with omeprazole and domperidone, whereas those in the treatment group were further added with Chinese formula. Questionnaire concerning reflux diseases was used to evaluate the symptoms before and after treatment. Two biopsy samples were taken from the lower esophageal for immunohistochemistry to evaluate the expression of CGRP and SP. RESULTS ①No evident differences were detected in the two groups when it came to the total scores of questionnaire concerning reflux diseases(P>0.05) before treatment. Both the total symptom scores and the symptom scores of burning, chest pain and reflux of gastric contents in the treatment group were lower than those in the control group(P<0.05), with no evident differences being detected in sour taste/bitter taste in the mouth(P>0.05) after treatment. The total curative rate of the treatment group was 75%, which was higher than 47% of the control group(P<0.05). ②Immunohistochemical test showed that the expression of CGRP and substance P in the treatment group after treatment was lower than that before treatment(P<0.05), while no significant changes were noticed in the control group(P>0.05). CONCLUSION The method of soothing liver, calming nerves, descending adverse qi and protecting membrane combined with western medicine therapy can efficiently relieve the symptoms and enhance curative efficiency in patients with NERD, whose action mechanism might be related to the inhibition of CGRP and substance P expression, so as to decrease esophageal hypersensitivity, protect the esophageal mucosa and increase its barrier function.

Abstract:
OBJECTIVE To characterize the homogeneous polysaccharides from Dendrobium officinale Kimura et Migo and study the hypoglycemic activity of the isolated polysaccharides. METHODS DEAE-cellulose anion-exchange chromatography was used to isolate homogeneous polysaccharides from crude polysaccharides (DOP); Monosaccharide composition was analysed after trimethylsilation; and the chemical structure of isolated polysaccharide was characterized with infrared spectroscopy; The effects of isolated polysaccharides on the levels of fasting blood glucose, plasma insulin, and glycosylated serum protein were estimated using diabetic mice models induced by alloxan. RESULTS Four isolated polysaccharides were isolated from DOP, in which DOP1 had the highest content. DOP1 was composed of arabinose, galactose, glucose and mannose in a molar ratio of 3.5∶1.2∶1.5∶1.0. The infrared spectroscopy of various isolated polysaccharides showed that they all had the obvious characteristic absorption peaks of corresponding monosaccharides. The levels of fasting blood glucose and glycosylated serum protein decreased obviously after a 21 day ig administration in diabetic mice models(P<0.01), in addition, all the four isolated polysaccharides increased glycosylated serum protein levels in varying degrees after a 21 day dosage (P<0.05~0.01). DOP1 displayed the optimal effects on the improvements of above three biochemistry indexes compared with the other three isolated polysaccharides. CONCLUSION Four isolated polysaccharides from DOP displayed hypoglycemic activity in diabetic mice models induced by alloxan.

Both commonly used herds and herb categories were summarized by applying frequency analysis with SPSS 17.0 and also clustering analysis was done to excavate the coreprescriptions and commonly used herb pairs. RESULTS The top three herbs on the frequency list of herb use were deep-fried Atractylodes macrocephala Koidz, Radix Pseudostellariae and Curcuma zedoaria. The commonly used herbs are qi-tonification herbs, heat-clearing herbs, qi-regulation herbs and herbs for invigorating blood circulation and eliminating stasis, with two core prescriptions and four commonly used herb pairs being excavated. CONCLUSION Professor Wang Canhui focuses on protecting the vital qi of spleen and stomach when treating patients with chronic atrophic gastritis and advocates the method of tonifying spleen qi, clearing away depressed heat, regulating qi and blood.

2016, 32(6): 592-594.
Abstract:
This paper summarized the clinical experience of Professor Chen Xiaohu in treating arrhythmia with the integrated Chinese-Western therapy based on his double heart concept. It is believed in Chinese medicine that the pathogenesis of arrhythmia is rooted in qi deficiency, yin deficiency and blood deficiency, with the branch being qi stagnation, phlegm, fire and stasis. Feeling ill at ease is considered as the center pathogenesis, with liver, spleen and kidney being involved. Hence, the therapeutic method is to calm the mind and nerves, together with soothing liver, regulating spleen and nourishing kidney, through herb medication and talk therapy. Such formulas as modified Shengmai Powder, Zhi Gancao Decoction, Ergui Pill are applied to treat arrythmia.

Abstract:
OBJECTIVE To investigate the clinical effects of combined treatment of Chinese and western medicine including Yinzhihuang, ursodeoxycholic acid and transmetil in the treatment of intrahepatic cholestasis of pregnancy. METHODS 80 cases with intrahepatic cholestasis of pregnancy were divided into observation group and control group, with 40 cases in each group. Patients in the observation group were administrated with traditional Chinese medicine Yinzhihuang and western medicine transmetil and ursodeoxycholic acid, while those in the control group were only given ursodeoxycholic acid and transmetil. Then clinical symptoms, changes of serum total bile acid(TBA), alanine aminotransferase(ALT), aspartate aminotransferase (AST), total bilirubin (TB), clinical effects and delivery outcomes were compared between the two groups. RESULTS Serum total bile acid, total bilirubin alanine aminotransferase and aspartate aminotransferase in the two groups decreased significantly after treatment and all these indicators in the observation group were significantly lower than those in the control group. The total efficiency of the observation group was much higher than that of the control group. There existed statistical significance in premature delivery, incidence of fetal distress, cesarean section, postpartum hemorrhage when comparison was made between the two groups(P<0.05). CONCLUSION Combining traditional Chinese medicine with western medicine for the treatment of the intrahepatic cholestasis of pregnancy exhibits excellent clinical effects and good perinatal prognosis, which deserves further clinical promotion.

Abstract:
OBJECTIVE To observe the clinical effects of Futong Xiere Formula from professor Huang Bao-zhong, a distinguished veteran doctors of TCM, in treating hepatic coma due to cirrhosis of liver and upper gastrointestinal bleeding through nasal feeding. METHODS 40 cases diagnosed with decompensated liver cirrhosis combined with upper gastrointestinal bleeding and hepatic coma in our hospital were collected from August, 2007 to December, 2012 and divided into observation group and treatment group, with 20 cases in each group. Patients in the control group were given conventional treatment including stopping bleeding, resisting hepatic coma, protecting liver and supporting therapy. Based on this, patients in the observation group were further given Futong Xiere Formula through nasal feeding. Then the clinical effects were analyzed between the two groups, together with hospitalization time and costs. RESULTS The total curative rate was 90% in the observation group and 60% in the control group. The observation group was superior to the control group in reducing total bilirubin and blood ammonia level(P<0.05). CONCLUSION Conventional treatment combined with herbal therapy through nasal feeding based on syndrome differentiation can efficiently stop bleeding, improve coma condition and enhance the results.
